Fibroid Treatments Show Similar 2-Year Symptom Relief

Laparoscopic radiofrequency ablation (Lap-RFA) and myomectomy have demonstrated comparable efficacy in providing significant and sustained symptom relief and improving health-related quality of life (HRQOL) for individuals with uterine fibroids over a two-year period. A recent study, published in Obstetrics & Gynecology, compared the outcomes of these two minimally invasive surgical procedures, revealing no statistically significant differences in key patient-reported outcome measures between the groups.

The study followed 166 women diagnosed with symptomatic uterine fibroids who underwent either Lap-RFA or myomectomy. Researchers assessed symptom severity using the Uterine Fibroid Symptom and Quality of Life (UFS-QOL) questionnaire, which measures bleeding, bulk symptoms, and pain, alongside overall HRQOL. At the 24-month follow-up, both treatment arms reported substantial reductions in fibroid-related symptoms and marked improvements in their quality of life. The UFS-QOL symptom severity score decreased by an average of 22.8 points in the Lap-RFA group and 21.1 points in the myomectomy group. Similarly, the HRQOL scores improved by 23.6 points for Lap-RFA recipients and 21.9 points for myomectomy patients. These improvements were sustained throughout the 24-month observation period, indicating long-term benefits for both procedures.

While both procedures yielded similar overall results, the study did note some subtle differences in specific symptom domains. For instance, the myomectomy group experienced a slightly greater reduction in pain symptoms compared to the Lap-RFA group, although this difference did not reach statistical significance. Conversely, the Lap-RFA group showed a trend towards better improvement in bulk symptoms. The study also tracked patient satisfaction, with both groups reporting high levels of satisfaction with their respective treatments. Approximately 85% of women in the Lap-RFA group and 88% in the myomectomy group expressed satisfaction with the outcome of their surgery.

Uterine fibroids are non-cancerous growths in the uterus that can cause heavy menstrual bleeding, pelvic pain, and pressure on surrounding organs, affecting a significant percentage of women of reproductive age. Myomectomy, the surgical removal of fibroids while preserving the uterus, has been a traditional treatment. Laparoscopic radiofrequency ablation, a newer technique, uses heat to shrink fibroid tissue. The findings of this study suggest that Lap-RFA is a viable alternative to myomectomy, offering comparable long-term benefits for symptom management and quality of life, potentially with different recovery profiles or suitability for specific fibroid types or locations. Further research may explore long-term recurrence rates and fertility outcomes for both procedures.
